Build a form

Choose a starter, add screens and fields, preview your form, and save a draft.

Build a form to ask customers for structured answers without leaving WhatsApp. Start with a ready-made set of fields or a blank form, then organize the questions into screens.

Before you start

Open Channels, choose your WhatsApp account, and select Flows.

Who can do this: the workspace owner or members whose role includes viewing channel connections and viewing and managing forms.

Steps

Choose a starter

Select Create form. Choose Blank, Lead capture (1), Appointment request, Feedback survey, Sign up, or Contact us. Enter Form name, choose Category, and select Open builder.

Organize your screens

In Screens, select the screen you want to work on. Edit Screen title (1). Use Add screen when you need another step. Customers start at the first screen; the visual builder takes them through the screens in order.

Use the arrow controls to reorder screens. Set Button text for the action at the bottom of each screen.

Add and edit fields

Select Add field (2), then choose a field type. Edit its question or text. For answer fields, turn on Required answer when needed and add helper text where available. For choice fields, enter each choice and use Add choice for more.

Use the field arrow controls to reorder questions. The remove control asks for confirmation before removing a field or screen from the draft. You cannot remove the only screen.

Preview and save

Review Form preview as you edit. Select Save draft (3) and wait for the save to finish. Fix any highlighted fields or errors returned after saving. Saving does not publish the form.

Choose a field type

Choose Short answer (1) for brief text or Consent checkbox (2) for a statement the customer can agree to.

Field typesUse them for

Limits and saving

A screen can hold up to 49 fields, including up to 3 images and 5 consent checkboxes. Dropdowns allow up to 200 choices; single and multiple choice fields allow up to 20. Each choice allows 30 characters. The whole form must fit within 10 MiB. Smaller images help; around 300 KB or less is recommended.

The builder shows Unsaved changes until you save. Before leaving, select Save draft. A leave warning helps you avoid losing unsaved work.

Good to know

Under an answer field's Advanced section, Field name identifies that answer in responses. The builder generates a name for you.

Forms with settings the visual editor cannot preserve open in Advanced: JSON. If switching to Visual editor fails, keep the existing form in that mode and ask someone familiar with its setup to help. Previews of dynamic forms can show placeholders.
